BreastCheck Informed Consent

Please see link below to the BreastCheck consent form which explains BreastCheck informed consent. You will be asked to sign this form at your BreastCheck Screening Appointment. Please note you do not need to print this consent form or bring it with you to your appointment.

When you attend for your appointment you will be asked to sign this consent form on a hand held device (or in some cases on paper where electronic consent is not available on the day).

It is helpful if you can read this information before your BreastCheck appointment. If you have any questions you can contact the phone number on your invitation letter or text message or you can ask BreastCheck staff on the day that you attend your appointment.
